Follow all of Cranbrook Institute of Science written and verbal procedures, including wearing a mask when within 8ft of check-in, upon entering, and while in the museum. I acknowledge, upon arrival at the museum, I will be asked If I have been in close contact with any possible or confirmed cases of COVID-19 within the past 14 days and may be asked to return after my quarantine period. I also confirm, on the day of my visit, nobody in my party is experiencing COVID-19-related symptoms.
Authorize Cranbrook Institute of Science to share my contact information, as well as the names of the adults in my party, with the Oakland County Health Department and/or State of Michigan Department of Health and Human Services if I/we may have been exposed to a potential or confirmed case of COVID-19.
Cranbrook Institute of Science (“CIS”) has put in place preventative measures to reduce the spread of COVID-19; however, CIS cannot guarantee that you, your child(ren), or those in your party will not become infected with COVID-19. Further, attending CIS could increase your risk and your child(ren)’s risk, or the risk of those in your party, of contracting COVID-19. I acknowledge the contagious nature of COVID-19 and on my behalf, on behalf of my children, and on behalf of those in my party, and I hereby release, covenant not to sue, discharge, and hold harmless CIS, its employees, agents, and representatives, of and from including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of CIS, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after visiting CIS or participating in any CIS program.
